Verified Statement of Claim for Lost or Damaged Goods

PLEASE READ THE FOLLOWING:

1. All claims must be filed in writing and received by Childs Express claims department within thirty (30) days of the date of delivery.

2. Claims for delivery damage will only be honored if they were noted on the Delivery Service Report at the time of delivery.

3. Do not discard or repair any items without prior authorization from this office, as we reserve the right to inspect all claims items and hire repair personal.

4. Describe the nature and location of the damage on the article.

5. Provide the manufacturer's name, the description, and the style number of any damaged items.

6. Supporting documentation such as purchase order and photos should be submitted with your claim.

7. Make certain that all items you wish to claim are included on your claim form and that the claim does constitute your complete and entire claim. Further additions to this claim once filed will not be accepted.
